Clinical Case Management System (CCMS)
Enhancing social support practices in schools through efficient access to data and tools for decision-making and accountability
Bringing together comprehensive diagnostic data, CCMS provides tools to help intervention teams in schools identify student needs and design appropriate social and academic support services. CCMS includes features to aid administrators in ensuring accountability. Designed for the Academic and Social Support System (AS3), CCMS also has potential general application as a decision-making and management tool.
CCMS introduces efficiency and evidence-based practice into student support services through:
- instant access to multiple forms of data assembled in student-level and class-level profiles;
- tools for planning and documenting student interventions and outcomes;
- supports for administrative oversight of and follow-up to intervention plans;
- communication functions for the exchange of information.
Problems of Practice
School professionals who strive to provide support services to individual students often find that student data is inaccessible and disorganized. They are hampered too because service interventions often go undocumented. These problems and others interfere with identifying students for referrals, making critical service decisions, and ensuring accountability.
